Be it Remembered, That on this day to wit: the SIXTEENTH of September in the year of our Lord, One Thousand, Ninehundred and THIRTY-THREE and the One Hundred and 58 of the Independence of the United States of America, before me, WM. H. ROBIN, M. D., Chairman of the Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in and for the Parish of Orleans, and the City of New Orleans, personally appeared

KELLY an undertaker native of this city residing 527 ELYSIAN FIELDS AVE, who hereby declares that SAMUEL RANLETT HUGHES (WHITE) a native of LA. aged 63 YRS. departed this life YESTERDAY (15 SEPTEMBER 1933) AT 3811 DAUPHINE STREET IN THIS CITY Cause of Death MYOCARDITIS, CHR. NEPHRITIS Certificate of Dr. H. W. ROELING. DECEASED WAS MARRIED TO MARIE DUZER, A LABORER, AND THE SON OF JAMES E. HUGHES AND MARY ANN JONES. FATHER NATIVE OF LA. MOTHER NATIVE OF GA.

Thus done at New Orleans in the presence of the aforesaid J. KELLY as also in that of P. H. Lanauze and W. J. Prudhomme both of this city witnesses by me requested so to be, who have hereonto set their hands together with me, after reading hereof this day, month and year first above written.
